2 forms of photo ID?

I'm updating the name on my account and the email I received with the instructions states:

1) A legible photocopy of government issued documentation of the name change, or of two forms of government issued photo ID showing your name before and after the change.

I only have my new drivers license. The kept and destroyed the old one. What do I do?

It appears there is a slight misunderstanding, Clothmi. The marriage certificate is not required to have a photo on it; we just need a photocopy of the document to show that your name has actually changed.

I would suggest sneaking a photocopy of your photo ID as well -- just to be safe. You may want to contact Billing for a more definitive answer, however.
