Trial of the Naruu: Mercy (Axe...?)

A'dal in Shattrath City wants you to recover the Unused Axe of the Executioner from the Shattered Halls of Hellfire Citadel.

This quest must be completed in Heroic dungeon difficulty.
** Unused Axe of the Executioner

Description
Four trials you must pass before you are deemed worthy of the Tempest key.

Within the Shattered Halls of Hellfire Citadel are three of your people. They are held there by the merciless one, Kargath Bladefist. He seeks to execute them as a sign of his virility and power. You must save these prisoners before such an atrocity can come to pass. As proof of deed, bring me the unused axe of the executioner. It must not have a drop of blood on its blade!

Okay so we do Shattered Halls heroic today and get saved to the instance etc.

Beat it in fast enough time to kill kargath and the executioner before time runs out (15 mins left on debuff!) And the executioner doesn't drop the unused axe?! Did he lose it or something?

-- No. Apparently the GM I talked to said "Its a high drop rate, but not guaranteed."

Sooo a quest item isn't guaranteed? Im confused this is a hard/intense instance and we don't get an unused axe off the executioner?

How would he execute these people if he didn't have an AXE?!

What happened to his axe?

Can you explain this to me why it isn't guaranteed and if it is guaranteed mind helping me out here... just a little bit please!

I've taken some time to look into this matter quite thoroughly, Ghelion, and I believe to have determined why the Shattered Hand Executioner did not drop the [Unused Axe of the Executioner] for your group; I can assure you that this was intentional, and not due to an errant bit of code.

Before I go into further detail, however, allow me to explain a small bit of in-game functionality that is relevant to this case. If there are ever more than five characters in a Heroic instance at the same time, you see, quest items shall not drop for any players for the remainder of the run. This becomes significant when you examine your group's efforts to complete "Trial of the Naaru: Mercy," which exhibited a couple oddities. To wit:

2007/03/30 04:26:37 GMT - Five players kill Warchief Kargath Bladefist in your Shattered Halls instance. You are not part of this group.

2007/03/30 04:27:58 GMT - A druid from your guild enters the Shattered Halls, after one of those original five has left the group.

2007/03/30 04:28:19 GMT - A warrior from your guild, and part of the original group, hearths out of the Shattered Halls.

2007/03/30 04:28:41 GMT - Another druid from your guild enters the Shattered Halls, after another original party member has left the group.

2007/03/30 04:29:52 GMT - A paladin from your guild, and part of the original group, hearths out of the Shattered Halls.

2007/03/30 04:30:49 GMT - A priest from your guild, and part of the original group, is unable to hearth. She logs out inside the instance.

2007/03/30 04:31:20 GMT - You enter the Shattered Halls, after another of the original party members has left the group.

2007/03/30 04:31:51 GMT - This new group, made up of two original members and three new ones, kills the Shattered Hand Executioner.

As we can see, one of the original members of the party was still inside the Shattered Halls (albeit logged out) when you joined the group and entered the instance. For this reason, the quest item you sought did not appear on the Executioner's corpse for those on the appropriate quest.

In closing, Ghelion, I would recommend against trying to circumvent the functionality of this quest by having five players clear the Shattered Halls, then summoning three other, unrelated players to the end of the instance (after three of the original members have left) so they can complete "Trial of the Naaru: Mercy" without having themselves invested the time to conquer a Heroic instance. As you can see, it doesn't always work out. :)
