Server wide problem with name reporting

Recently on our U.S. The Venture Co. (RP-PVP) server, there has been a severe problem with over-reporting of both character and guild names that "violate" the RP naming guidelines. As of late, a substantial amount of Horde guilds and active members of the RP community of either faction have had to deal with forced account actions and appeal processes for what many to consider unnecessary reasons.

What is usually used as a tool to police the RP nature of the realm to keep player characters and their guilds from seeming "out of character", has become a giant nuisance and an over-abused problem. The worst part of all, is that it seems no character or guild name, no matter how original, unique, or blatantly IN character, is safe from being prompted for a name change.

I was wondering if some GM's here can shed some light on both a) the process that goes into prompting a player or guild to change their name and b) how a player who feels they were thoughtfully and actively rping can contest accusations of violating the RP policies. Any and all insights you could provide would be very helpful in starting to deal with this server wide problem.

Can I get a list of some names that got changed, to sate my ungulate curiosity?

Uh, Frosska's name wasn't changed, nor is there any action on the account for anything involving naming. Where is this information coming from?

Report does not equal name change, though! Heck, someone can report my name if they want, it doesn't mean it's going to get changed. Name changes aren't at player discretion.

If you guys are worried that someone's name was unfairly changed for Non-RP, please let me know their current name and I'll look into it.

Not unless it was a "LOL REPORTED" deal where the reporting player simply couldn't keep it to oneself. We don't inform the player of reports against them, only actions we'll be taking against them.

All I can figure is that it must've been sent accidentally, because your name isn't in any danger of being changed. You also haven't been actioned or suspended.

Looks like this is all a big misunderstanding.
