It appears that the first two faxes were no rejected as a result of the form being illegible, but were not notarized appropriately. Only the third attempt was deemed illegible by our
Account Administration department; however, this was months ago.
I suggest resubmitting a fax and mailing the documents to the locations mentioned within the document. If this is unsuccessful, I suggest contacting Account Administration asking for more information as to why your application was illegible. Regrettably, this is information that only the Account Administration department is able to provide.
