Account Retrieval form - Blue Tracker

I can't seem to find a link to the Account Retrieval form anywhere on the website, I sent one in but apprently I'm missing something :(.

I went to the link I got given in the email from blizzard, but its very.....vague

any help would be appreciated. thanks
