I have the squashling and the helm however I do not have the achievement for it and when I check it there is only the squashling listed. I am assuming there is a glitch but can you tell me if this will be fixed. Thank you

Unfortunately, since the Helm could be traded last year, and data records often don't go back that far, there may not be any way for Blizzard to verify how you got the Helm you have. (You'd have to loot it from either the Horseman / Loot-filled Pumpkin, or one of the Trick or Treat Bags, or the Matron's gift for it to count.)
