4.01 upgrade toon phased out

4.01 upgrade resulted in 1 of 10 toons, stuck in a world of no mobs, no animals, no portals, no boats, nothing alive but the toon itself. All of my other toons are fine.

Got an email saying it was my UI and to rename 3 of my folders to "WTF-Old," "Cache-Old," and "Interface-Oldxxx -old. and restart the game, this didn't resolve the problem.

I re-formated my hard drive with windows 7 ultimate 64 bit, reinstalled my antivirus kaspersky internet security, reinstalled wow, and this hasn't resolved the problem.

Not sure if there is anything else I can try at my end. It can't be my UI if all my other toons work perfectly. And this one doesn't.

When the server shutdown I was cleansing the Battered Hilt in the Well (part 9 or 10) of the battered hilt quest chain on the isle of quels. I was inside the instance. When the acct logged on, I was no longer in the instance or even the isle of quels, but Dalaran. There was nothing anywhere on the entire northern continent to talk to, kill or leave the continent.

After I turned in a report, I was outside Ironforge, and the same scenerio, no npcs, players, anything.
