Changing Parental Control Password

Hello, I am having trouble understanding the exact guidelines as to how to do this. I have already found the form capable of doing this, http://us.blizzard.com/support/article.xml?locale=en_US&articleId=26361, but am lacking a mailing address to send said form to.
Also for proof of identity, is a photocopied picture of my driver's license okay? I would hate to have it rejected and make a second trip to the post office to mail a second letter.
Lastly to clear up any further misconceptions I have, does the entire form does not need to be filled out? I assume it is only the parts that apply to my situation.

I'm a little confused on exactly what you are needing to do, Superbia. Changing the password should be accessible to you through the Parental Control e-mail set up on your child's account (if this is, indeed the correct assumption for the situation).
