Forcefully renamed toons

I happened to catch a post in General before it was deleted, about someone upset that someone kept reporting their name for being offensive. Now, this isn't directly related, but it got me wondering...

When someones toons get "flagged" for renaming (offensive or otherwise), why are some users given the option to rename them while others arent?

In this case, my mage (Blackrock, Alliance) was named "Shortbuspimp". Now I don't really see whats offensive about it, but whatever, Blizzard agreed with whoever found it offensive (I even made a nice little email for one of my other toons, since it was based off of a family name--which thankfully hasn't been messed with since I sent that email).
Anyways, It was never given the chance at a rename, and I log on one day and find her renamed as "Ganena". Now, as I mentioned earlier I've had another toon who was renamed, but this toon gave me the option to pick a new name and not some name that was picked for me by some blizzard GM or what have you.

For what it's worth, this other toon wasn't exactly named appropriately as I was being lazy and not using the special characters that are on our family's Coat of Arms... if you wanna be technical about that name.

Now, I'm not trying to get my toons renamed, as this happened early this year, but I would like a reason why some people's toons aren't given the option of a self chosen name while some others are forcefully chosen. It appears at a glance that this is a shady business practice trying to get people to use the paid name change service.
Then again it could have been a mistake. Or in reguards to myself, been because of my accounts history.

With so many accounts I doubt blizzard would investigate and research some of these "offensive" names, such as my toon using the family name despite it appearing as offensive.

Simarly related: X-Box lIve suspended a guy for having the word "Gay" in his profile. Fort Gay, West Virginia was the town he lived in. When he was suspended, he brought it up to Live administration, but they just ignored him because the word "gay" was being used and instantly thought he was lying and wouldn't even google it. This guy has since been cleared and back doing whatever he does with x-box live.
Only philisophically related.

Looks like the reason you were given a name was because this was the third time we had to action that specific character for very similar names with minor alterations.

If a player is showing that they lack the capacity to pick an appropriate name, then our Game Masters will take it upon themselves to pick one for them. While we like to be lenient with players who have never been actioned before, this was not the case in this situation. Next time, you will want to review our naming policy before again losing the ability to pick your own name.

This thread's become a bit circular, so let's give it an end.
