Instance lockouts

Just a quick question about the rule concerning the limit of 5 dungeon entrances per hour.

How does the game mechanics handle deaths and having to run back inside? Does this entrance back into the dungeon count against the limit?

I ask because twice now on two separate alts, we've had trigger happy tanks who rushed in and started the Anub encounter in AN before everyone was inside the ring. (People drinking/eating, looting, etc.) Of course, those of us stuck outside died and had to release and run back inside after loot was rolled on and party had disbanded leaving us with no choice.

(I'd hate to think this was counting against us, because there's not a thing we can do in this case.)

An instance resets when you create a new instance of an instance (either by zoning in the first time, or by zoning out and resetting, and then zoning back in afterwards).

The LFD tool (both for regular instances and for special events) seems to bypass this restriction.
