Getting back a deleted toon after 9 months.

So my problem is, I decided I wanted to break myself from World of Warcraft and by doing that I would delete my level 80 warlock. I deleted her about 9 months ago thinking I wouldn't play again. I started up again around 2 months ago because I really did want to play, but after a short time I really have regretted my foolish mistake in getting rid of a good character. I also just found out recently that I could have a chance to get my character back if I send in a form with some information on it. I filled out the form and the character's name, race, level, class, etc... The only problem is, I put my account's password instead of my account name. So can someone please give me some sort of hope in getting my toon back by explaining to me how, if possible, to get it back? Even after screwing up so many times as I have.
