Unauthorized Authenticator

My friend has had an authenticator put on his account but he did not put it on. I've heard of this happening a lot but have never known anyone who it happened to. What does he do?

Well, I do see this reported, so that process has started for an investigation - but finding the source of this is going to be vital to it not repeating.

You may also want to look into getting an authenticator for your account. It's no substitute for good security habits, but it will help keep them out of your WoW account.
