My quests are glitched :(

I know for a fact that my quests are glitched somehow, because my Objectives tracker says I'm tracking 1 more quest/achievement than I am at all times. When I'm not tracking anything it still says Objectives(1) and has that collapse button (though the button is greyed out).

I'm wondering if this has anything to do with my inability to get the Hinterlands OOX beacon. Normally I get this beacon while questing in the area. Now with this character, I've mostly cleared Jintha'Alor like 5 times now and it still hasn't dropped. I know the drop rate is low, but it is not that abysmal.

Can anyone please help fix these issues? For the OOX one I would at least like a confirmation on whether or not the drop is glitched for me.

I took the liberty of logging into your character, Ryume, and when looking around, I wasn’t able to see the root of the cause for the tracking issue. That being said, I have a feeling it may be related to a third party AddOn. Please try logging in at your earliest convenience, and let me know if you observe anything being tracked; they should all be disabled at this time, from my tinkering.
