Couple of questions

You are certain that when you are attempting to set up your Parental Controls that you are entering the Battle.net account name and password? It won't be the old World of Warcraft account name.

From what I can see the Parental Controls are not set up on this account.

Perhaps the issue is with the browser itself? Perhaps clearing your temporary internet files (cookies/cache) or trying a different browser?

If this is on another account please give me the name and realm of a character on that account and I'll take a look.
