Quick question on exchanging my Authenticator

I printed off the order form that I need to fill out, I was just wondering do I put the Authenticator in the envelope that I mail the order form with? Or do I just mail the order form and then I'll receive something in the mail to put the broken Authenticator in?

I believe you'll want to send the broken Authenticator but I am inquiring to see for certain. I'll let you know as soon as I have an answer. :)
