The information that you included above should be more than enough to get the situation taken care of, Cetan. It sounds like the Billing & Account Services representative was slightly confused about the situation, as their information sounds more in line with a compromised account, which is not the case in your situation. All that is required from you is the above information included in an in-game petition. The Game Master staff will evaluate the situation and do everything in their power to restore your character to its former glory.
