Tried the "custom lag tolerance", but it did nothing to remedy the problem. Tried raiding today, and it was incredibly frustrating. My HotStreak would proc, and I'd have to hit it 5 or 6 times before it would cast. Sometimes I wouldn't even be able to get it off. Other times, I'd cast fireball, and nothing would happen....then 5 or 6 seconds later, fireball would magically start casting. I've tried everything. I re-installed WoW, updated Windows, Updated my video card. What else can i do? I'm honestly considering canceling my account. I hate that I'm so frustrated after sitting down to do something I used to enjoy.
This is very strange. I'm racking my brain as to what could be causing a distinct issue like this for just a certain set of players, but nothing is jumping to mind. Let's start with a networking long-shot, and assuming that doesn't work we'll start again from the ground up with some DxDiags.
First up, the longshot. Assuming you're on Vista or 7, try switching off IPv6 in your network controls. Handy Microsoft instructions to finding the toggle is here:

For Windows 7:
Important! - Please reboot your system prior to creating the dxdiag file.
1. Press the Start button.
2. Type "dxdiag"
3. Select the "Dxdiag" link that pops up.
4. Click on the 'Save All Information' Button (save this file to your Windows desktop and name it dxdiag).
For Windows Vista:
Important! - Please reboot your system prior to creating the dxdiag file.
1. Press the Windows Key (the key with the Windows logo on your keyboard) + R (at the same time).
2. Type in DXDIAG and press Enter or click 'OK', this will open the DirectX diagnostic tool.
3. Click on the 'Save All Information' Button (save this file to your Windows desktop and name it dxdiag).
For Windows XP:
Important Note: Please reboot your system prior to creating the dxdiag file.
1. Click on Start.
2. Click on Run.
3. Type in DXDIAG and press Enter or click OK, this will open the DirectX diagnostic tool. Run the DirectDraw, Direct3D and DirectSound tests found under the Display and Sound tabs before creating the dxdiag.txt file.
4. Click on the Save All Information Button (save this file to your windows desktop and name it dxdiag).
