How long does it take remove authenticator

I was a bit confused about the directions to. It said to sign the TOS form which I did, but did I have to actually print off the TOS and sign that to?

The Terms of Use form needs to be completely filled out and signed to be valid. You must also include the requested proof of identification in order for it to be processed properly. There can be a bit of a delay in response as all faxes are handled in the order received. Keep an eye out on your registered e-mail address as they will follow up with you there once your form has been processed.
