Unlinking a wow account from a bnet account

Long story short, a hacker created a trial BC account and merged it with my battlenet account. The account was banned and now the trial expired, but it's still linked to my battlenet account. I emailed Billing and Account Services and they said I had to fill out the TOU form. Problem is, on the TOU form there's no choice to choose to unlink a wow account from a battlenet account. Am I supposed to move my wow account to another battlenet account or is their a way to unlink the trial wow account from my battlenet account? Thanks in advance =)

TOU form in case it helps.

After reviewing your account it does become clear that second account should not be there, Lindsy. I have gone ahead and had it removed so it doesn't show up on your Battle.net anymore. Or, more bluntly, "Get out of that other persons Battle.net account you silly WoW account, you don't belong there!" :)

In case you haven't taken steps to secure your computer yet, I do think our Computer Security Recommendations page is a good read and worth your time.
