Players first entering any dungeon that will result in a lock-out timer will be warned if they are going to be saved to the instance. This warning will include an option to port to the nearest graveyard before being saved.

In theory that should be the case. I haven't tested it myself or read through any bugs associated with it.

Again, I haven't tested it. My understanding is that, if you're in the instance and eligible to receive emblems, you will receive them automatically as soon as someone loots them. If you are not in the instance and eligible for the emblems, it will remain on the corpse until the loot is retrieved or the corpse despawns.

because that does prevent us from getting saved to "fresh" runs that arent fresh, but often you join knowing boss X is down, and then find out that boss X, Y, Z, A, D, E, and F are down, and you have boss O and G left.

It does not do this. It's definitely something we're interested in doing. I don't know off-hand if the tech for that exists. Either way it's something that would take a bit more time to develop. With the lock-out warning going into 3.1, the goal is at least to help curb the growing issue of players being saved to instances they thought were fresh. It's not going to solve all lock-out issues -- like you said, there could be fewer bosses alive in the instance than you were led to believe -- but it's a step in the right direction.
