The Naked Glitch - You've seen it too, I know

Ok, since we have everyone's attention while we wait for the server's to finish their maintenance cycle, can anyone tell me anything about the 'Naked Glitch' ever since WoWLK? It happens way more than I'd expect to see and a lot of others have seen it routinely too. Does anyone know what triggers this? It seems like it's only related to instance runs but I've come in quite a few times to a shock of all my party members in nothing but their skivvies. Of course, they see their gear and others as well, but not me. At the same time, I've been on the other end of the deal too where I see everything just fine but someone else sees only the nakedness of us all. It never affects stats, it's just the visual and a UI reset usually fixes it. Anyway, just thought I'd kill time and ask.

Not sure what causes this either, but I have seen it myself.

Zoning out and back in also normally resets this.
