Stratholme: How does I spawn Ramstein?

I did Stratholme twice earlier.

I destroyed the three ziggurats and cleared all of the abominations in front of the Slaughterhouse.
the problem is, Ramstein the Gorger doesn't spawn, so I can't finish the instance.

what am I doing wrong?
