Some quick answers:

* Your MMR transfers realms with you if you move your character to a different realm.
* Your MMR does not reset if you delete and re-make your team.
* Your MMR does not reset if you leave one team and join a different one.
* You have a different MMR value for each bracket (2v2, 3v3, 5v5) that is independent of each other.
