Missing achievements

Before WOTLK and achievments were brought in, I completed loads of things listed now in the achievments.
I had a long break just before expansion was released and on returning noticed that some of the old things I did in Vanilla and BC are updated and some are not.

Firstly, how come it has not updated correctly?
And can a GM update it from my account details?

Some items will flag you for achievements you earned previously. For example if you had a BoP item from C'thun when the achievement system was implemented, you would be given credit for the kill.

If you had killed C'thun once before, but do not have loot to prove it, the achievement system would not know and cannot credit the kill. That is an example and the common cause for not having achievement credits for achievements earned way back in the day before the achievement system existed.
