Google Authenticator - QR Code "Not a valid token"

So, I got a red banner suggesting I change my password. Then I see on the My Account page that I can use Google Authenticator.

So, I download the app, hit Next and try scanning the QR code. No matter how many times I refresh or go through the steps, Google Authenticator always says the QR code is invalid.

Hi Beltaine,

In addition to Entity’s suggestion, can you tell me what type of phone you’re using? We’ve seen intermittent problems with the QR codes and any details we can get that help narrow down the issue would be highly useful.
