The first or second rare item I received while playing was a "fallen tooth". I thought this was odd that I found one drop at all because the description stated it came from braking down rares.
I thought little of it and placed it in my stash, but I have transferred it back and forth between the stash and my inventory only a couple times and found something weird happening today. I looked in my inventory and found it was there after I remembered stashing it. I thought this odd and went to put it back and found I already had one in there just as I remembered.
I dismissed it and assumed that I had somehow picked up another dropped tooth and just stacked them in the chest.
Then it happened again!... It was in the exact same spot (near the bottom left of my inventory, not in a place it would land be default) I can see forgetting or something once, but not twice? I know this seems truly far fetched, and I really hope it is just a paranoid tester being dumb...
I have only broken down one item that was not a blue or white. It was a Griswold Edge (I had two). I am fairly certain that I would remember breaking down a rare, and I'm really careful about what I click on...
If someone would like to look at them I took a ss (for what that is worth) and have resisted the urge to use them for crafting. Maybe items have some kind of unique id that can be checked? I hope stacking them didn't ruin anything a dev could look at.
