The first time I took notice of it was on my Monk after killing Jondor. He dropped a "Club of Thrashing", and when I moused over it the thing just disappeared completely. I thought that perhaps I accidentally clicked on it, so I checked my inventory and it was nowhere to be found. Each time this happened there was more than one Magic item on the ground.
I'm sure this has occurred more frequently than what I've managed to catch, simply because when you mouse over something you typically click it right after. So as it vanishes you aren't consciously thinking to yourself "Oh wow it just poofed", you are subconsciously thinking "That (insert item) is now in my inventory because I just clicked on it, that is why it's not there anymore".
The way I was able to catch the other occurrences was by delay clicking on magic items I found off of Jondor and chests, just to see if it would happen again, and it has.
Another thing that might be worth looking into is spamming spacebar during the Jondor event to make it go faster. I often find myself hitting it rapidly to make the Templar kill Jondor, close the dialogue, accept the quest reward, and clear the second path. That might have some connection to what is happening there, but it doesn't have any connection with the chests.
Hopefully I'll be able to reproduce this in a reliable and less random way so it can be weeded out before live. Because I can only imagine how infuriating it would be to see some really nice item vanish, just as you went to pick it up off of the ground.
P.S. It also happened once during the hunt for the Blacksmiths wife. It was a Ring that time but I can't remember the affix.
