I was just playing single player and after meeting Leah the first time, in the inn, i went into her room clicked on her book and read her journal entry 1. However if you never go back to read the journal until after rescuing Deckard Cain when you click on the book in her room it skips right to journal entry 3, about being glad her uncle is back. Leaving the room and coming back in the book on the desk spawns but when interacted with plays the falls to the ground animation but spawns no new journal entry. Have had this happen to me on a monk and wizard and would assume it happens on all classes.
