It would be helpful to have a Horadric "undo"

I've not done it yet, but I'm sure the day will come when I accidentally turn my Ultra-Unique-Rare-Socketed-Enchanted-Super-Sword of Awesomness into common scrap.

The confirmation dialogue on rare and above, as others have mentioned, is our mechanic to warn people. We don't have any plans to introduce additional measures as the ability to 'buy back' a randomized output is actually fairly complex. When you sell an item to a vendor we know how much gold you got for it, if you want to buy it back we make sure you have that much gold, and viola we exchange it back. When you salvage an item what you get isn't a set amount or reagent, you could get a mix, and tracking that each time someone salvages an item for all characters in all regions isn't a super simple design (or even service) problem. Obviously not an impossible one, but it's significant enough that we have to weigh it against other features and make a decision. The decision, for now, is to rely on the warning.

Be careful.

It's also prone to exploitation. If they don't like the mats from the first salvage, just undo it and try for better ones next time! An undo is not an option.

Also a good point, the tracking would have to ensure re-salvaging wouldn't actually roll up new results. As I said, not impossible, but certainly not trivial to design and implement.
